As a Pharmacist at AmeriMed you will access patients’ suitability for home infusion care, review and evaluate medication orders, labs and other clinically relevant information monitoring patients’ progress and making recommendations when needed.   Key Job Responsibilities - Assess patients’ suitability for home infusion care in accordance with Infusion’s policies and procedures. Knowledgeable of medical conditions and prescribed medication therapies suitable for home care services.  - Monitor and evaluate patient specific pharmacy care plans for the pharmacotherapy of home infusion patients. Collect and organize all patient related data needed to prevent, detect and resolve medication-related problems. Design and modify pharmacy care plans based on evaluated information and make recommendations for continued monitoring when appropriate. - Receive medication orders from physician and evaluate the appropriateness of orders for dosage, drug choice, potential drug interactions, drug-food interactions, route of administration and stability. - Ensure medication specific laboratory tests are ordered and results received. Responsible for the retrieval, review and consultation with physicians and nurses of ordered labs. - Manage the use, maintenance and troubleshooting of all pharmacy equipment. Ability to accurately program, maintain and troubleshoot all IV medication related equipment. - Provide medication-related education to Infusion staff, nurses, physicians, patients and caregivers. - Assist the physician, nurse, patient, caregiver and Infusion staff with continued technical and clinical support. Provide accurate, concise and timely responses to clinical questions from health care providers, patients and caregivers.
