We’re seeking a 24 hour part-time MSW to join our team!       - Provide services for patients in their own homes! - Have the autonomy to create your own flexible schedule! - Work with patients around where you live! - We’re moving to a new Homecare Homebase electronic medical record that will save you time documenting!       Responsibilities - Provides resources and education to patients and their caregivers - Participates in the coordination of services with other disciplines and community resources as needed - Evaluates and revises care plans/assessments based on changes in patients and/or their environments - Participates in regular case conferences and in-services or special case orientations as needed
