AmeriMed in Louisville, KY has provided home infusion therapy services to help our patients safely transition care to their home for over 20 years. At AmeriMed we assist patients recovering from surgery, being treated for an infection, or with nutritional needs to maintain an active, healthy lifestyle while receiving care at home. Through our experienced and professional pharmacy team at AmeriMed we provide total care for our patients. Working directly with physicians, we are able to manage a variety of diseases and conditions in a homecare setting, alleviating long and unnecessary stays in the hospital.



AmeriMed Home Infusion Pharmacy in Louisville, KY is looking for an Care Transition Coach to join our team!


As a Care Transition Coach at AmeriMed you will:

  • Develop and maintain relationships with referral sources including but not limited to Care Coordinators, Managed Care Case Managers, and Physician Offices regarding Infusion services.
  • Support marketing efforts and meet or exceed established monthly admission target(s).
  • Maintain knowledge necessary to serve as an educational resource for referral sources regarding product and payor coverage for infusion therapy, enteral therapy and durable medical equipment.
  • Follow referral of a patient to the organization and assists discharge coordinators in coordinating quality home care services for patients
  • Determine patient eligibility for Infusion services.
  • Determine if a patient would benefit from Infusion services.
  • Verify insurance coverage.
  • Explain Medicare and Medicaid guidelines, insurance plan benefits, financing options, and organizational policies.
  • Observe and report trends, changes, and identify new opportunities for business.


  • High school diploma or equivalent.
  • Three years of experience in home infusion admissions is required.
  • Sales or marketing experience preferred.
  • Customer service focused.
  • Knowledge of home care regulations.
  • Strong assessment critical thinking, judgment, creativity and decision making skills.
