Deaconess Home Health has provided home care and wellness services for many years. Our innovative services and specialty programs recognize the needs of each patient and promote the highest quality of life. Our goal is to ensure all services consistently exceed quality measures and the expectations of patients and referring physicians. Deaconess Home Health’s quality care is provided through a team of registered nurses, social workers, companions, dietitians, aides and physical, occupational and speech therapists. We serve the counties of Posey, Vanderburgh, Warrick, Spencer, Gibson, Pike, Daviess, Dubois, and Knox.


The Physical Therapist develops, implements, and supervises the home care plan for the physical rehabilitation of patients, under the direction of the attending physician and clinical branch leadership.


  1. Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information. Maintains confidentiality of patient information.
  2. Performs initial evaluations and establishes care plan, including environmental modifications, for continued needs based on the patient assessment. Performs physical therapy treatments according to the plan of treatment. Evaluates and revises care plans and assignment sheets based on changes in patients and the environment.
  3. Reports changes in patient condition to appropriate personnel and the attending physician.
  4. Provides patient care education to patient and other caregivers.
  5. Participates in coordination of services with other disciplines and community resources as needed.
  6. Provides paraprofessional supervision as required by policy and regulations. Provides supervision for physical therapy assistants (PTA) as prescribed by state and federal regulations. Visits all patients under PTA care every thirty days.
  7. Completes and submits all required documentation in an accurate and timely manner.
  8. Demonstrates sensitivity to patient and/or caregivers needs, customs or feelings.
  9. Participates in case conferences, summary reports, quarterly record review, annual program review and in-services or special case orientations.
  10. Participates in on-call rotation, as required.
  11. All other duties as assigned.


  • Bachelor’s degree from an accredited school approved by the American Physical Therapy Association
    Currently licensed to practice in the state serviced
  • One-year physical therapy experience in teaching or clinical setting
