If so, CHI Health at Home is looking for a Nurse Coordinator I to support our Field Staff at the Hot Springs, Arkansas Branch. Where you will acts as initial service ambassador to referral sources, physicians, patients, families and other external customers providing the highest quality service.


Some job functions listed, but not limited too:

  • Responsible for obtaining authorizations, benefits coverage and requested updates for continuation of services. Notifies Director, Clinical Services (DCS) as needed to ensure necessary information and documentation is obtained.
  • Completes the intake referrals according to established processes ensuring complete, accurate timely information.
  • Acts as a clinical intermediary for the field staff
  • Ensures the processes for physician orders (485’s), recerts, OASIS assessment data entry, updates and changes are completed in an accurately and timely manner.
  • Completes clinical documentation, (i.e. discharge summaries, etc.) according to organizational polices. Assists the DCS in review of processes and paperwork as requested.
  • Completes patient visits as needed and as direct by branch leadership.
  • Assist with Telehealth program as directed by Director, Operations or DCS.

  • Current LPN license in good standing to operate in the state serviced. Experienced working in Home Care and/or Hospice a plus
  • Excellent organizational, interpersonal and communication skills.
  • Good computer skills, familiar with multiple software programs including Windows based programs.
  • Excellent organizational, interpersonal and communication skills.
  • Strong clinical skills.
  • Ability to work closely with a wide range of professionals or a multi-disciplinary team.
  • Interpersonal skills that facilitate inter- and intra-team/departmental communication. 
  • Excellent organizational and time management skills.
  • Excellent attention to detail is required.
  • Valid Driver’s license and auto liability insurance coverage according to organizational policy is required.
  • Compliant with appropriate state certification/registration and organizational requirements as necessary
