Calling all experienced State Licensed CNAs, are you looking for rewarding Nursing Career path where opportunities to grow within the company is available?

If so, CHI Health at Home is looking for a CNAs to work in in Home Health care, where YOU MATTER. Where you will work as a Home Health Aide asisting in daily personal and grooming services, household care and provision of nutrition. This position will work under the supervision of a Registered Nurse, Physical Therapist, Occupational Therapist or Speech Therapist.

Other duties include but not limited too:

  • Assisting with ambulation, and assists with active or performs passive range motion exercises.
  • Communicate changes regarding patient’s physical/mental condition, completes and submits documentation in a timely and appropriate manner.
  • Participates in case conferences, attends in-services, and/or special case orientation.

  • High school graduate or equivalent.
  • CNA-Completion of 75-hour, 120-hour or 150-hour Certified Nursing Assistant Program and must be on State C.N.A. Registry. Graduate of an accredited RN program may be substituted for completion of CNA program and CNA Registry.
  • One year of Certified Nursing Aide-Home Health Hospice work experience a must.
  • Must be dependable, with reliable transportation
  • Must have valid Drivers License and show proof of active auto insurance
  • Must be able to communicate with all level of medical professions
  • Must obtain and maintain CPR or BLS certified.
  • Successful completion of all competency testing.
