Now seeking a full-time LPN to see patients in Scott, Bourbon, and Clark Counties!

VNA Health at Home/Lexington is looking for a full time LPN who is compassionately committed to providing quality care and service and has at least one year of clinical experience.

Responsibilities:

  • Provides care and education to patients and their caregivers
  • Participates in the coordination of services with other disciplines and community resources as needed
  • Evaluates and revises care plans/assessments based on changes in patients and/or their environments
  • Participates in regular case conferences and in-services or special case orientations as needed and participates in on-call rotation

Qualifications

  • Graduate of an accredited school of nursing
  • Currently licensed to practice in Kentucky
  • One year nursing experience required
  • Must obtain and maintain current  CPR certification (all locations)
  • Proof of eligibility to pass medications required
