CHI Health at Home in Grand Island, is currently seeking a Full-time experienced Care Transition Nurse (CTN) to cover Grand Island, Nebraska area. The Care Transition Nurse (CTN) acts as a field-based business development representative and is responsible for growing the market and meeting the expectations for referrals and admissions. They observe and report trends, changes, and new opportunities in the market and identify ways to increase awareness, educate and promote home infusion services.


Job responsibilities include:


The Care Transition I (CTN) collaborates with physicians, hospitals, nursing homes, home care and hospice personnel in the process of providing coordinated quality home care services for patients referred to CommonSpirit Health at Home. This activity may be in a health care facility or in the field office location. The Care Transition Nurse observes and reports trends, changes, and new opportunities. Increases awareness among key contacts at health care facilities when home care and/or hospice is an optimal solution and helps identify when services are appropriate for individual patients. Assists patients in making a seamless transition to home care.

  • Be a graduate of an accredited school of nursing, with a current RN or LPN license in Nebraska.*Obtain and maintain current CPR/BLS certification
  • Experience as a Care Transition Nurse or similar role, sales or marketing, or customer service.
  • Have a minimum of Two years clinical nursing experience. Hospital or home care nursing experience is preferred. Home Infusion experience is a major plus, but not required.*
  • Knowledge of home care regulations is required.
  • Results oriented professional, self-motivated exhibiting strong initiative, vitality, and follow through.
  • Excellent communication skills. Ability to engage, connect, and tailor message to audience. Effective communication is key to success in this role.
  • Exhibits professionalism in daily tasks and ability to build strong relationships and trust with referral sources.
  • Must be a critical thinker, with good judgment, creativity and decision making skills.
