MercyOne Home Infusion in Des Moines, IA is currently seeking a full-time Care Transition Nurse (CTN) to cover Iowa City, IA. The Care Transition Nurse (CTN) acts as a field-based business development representative and is responsible for growing the market and meeting the expectations for referrals and admissions. They observe and report trends, changes, and new opportunities in the market and identify ways to increase awareness, educate and promote home infusion services.
The Care Transition Nurse focuses on cultivating relationships with referral sources which include area hospitals, rehab facilities, nursing homes and physicians. Helping to educate them on the variety of services offered by our company, increasing awareness on the benefits of home care, and identifying how and when services are appropriate for individuals and would provide an optimal solution for the patients needs.
The Care Transition Nurse is a general home care resource and provides value added counsel, teaching and resourceful problem solving to the patient, health care provider and facilities. The CTN aids in the referral process by determining eligibility, explaining Medicare & Medicaid guidelines, explaining insurance plan benefits, financing options, obtaining verbal or written orders, communicating with appropriate disciplines involved in the care, and providing education to the patient to assist patients in making a seamless transition to home infusion services.

*In substation for post high school level education and nursing experience candidates must have five years IV/Home Medical Equipment customer service experience and a Medical Terminology certification to be considered.
