Community Mercy Home Care is looking for PRN RN's to join our team!

As a Registered Nurse at Community Mercy Home Care you will:

  • Be at the forefront of providing homecare and hospice nursing services to patients who are rehabilitating, managing chronic conditions, or are in the end stage of life.
  • Provide care following the plan of treatment and patient care plan.
  • Establish a care plan for continued needs based on the client assessment.
  • Evaluate and revises care plans and assessment sheets based on changes in patient and/or the environment.
  • Report changes in patient condition to appropriate personnel and the attending physician.
  • Participate in coordination of services with other disciplines and community resources.
  • Provide patient care education to patient and caregivers.
  • Be a clinical resource for patients, caregivers, as well as other external customers. 
  • You will provide quality services to patients in a compassionate, professional, ethical and knowledgeable manner, thereby creating high levels of customer satisfaction and successful outcomes!

  • An active RN license in the state of Ohio
  • Must obtain and maintain current CPR certification.
  • One year of RN Nursing experience or three years of LPN Nursing experience providing direct patient care in a home health setting.
