Community Mercy Home Care is looking for PRN LPN's to join our team!

As a Licensed Practical Nurse at Community Mercy Home Care you will:


  • Collaborate and coordinate patient care with designated RN Case Manager and other disciplines focusing on the right care at the right time for the right outcomes.
  • Make skilled observations regarding patient’s physical and mental condition.
  • Report changes in patient condition to appropriate personnel and the attending physician.
  • Provide patient care education to patient and caregivers.
  • Participate in coordination of services with other disciplines and community resources.

  • Currently licensed to practice in Ohio.
  • Minimum of 1 year clinical experience required.
  • Hospital nursing experience required. 
  • Must obtain and maintain current CPR certification.
  • Proof of eligibility to pass medications required.
