Team Coordinator/Intake


Position Summary:
The Intake Coordinator is responsible for the complete, accurate, and timely registration and initial coordination/scheduling of clients to the company’s services while maintaining exceptional customer service.

Key Responsibilities:

Supports CHI Health at Home’s mission to exceed the expectations of our customers, associates, and shareholders in the delivery of health care and support services in a way that a caring family provides. Follows the policies and procedures of the organization.
2. Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information.
3. Acts as initial service ambassador to referral sources, physicians, clients, families and other external customers providing the highest quality service.
4. Responsible for the complete, accurate, and timely registration of clients to home care/hospice, including but not limited to demographic information, payer information, service needs, etc.
5. Responsible for the initial entry, verification, and maintenance of information regarding new clients in all applicable computer software programs.

Qualifications

 

  • Bachelor’s degree, or any equivalent combination of education and.
  • Excellent verbal communication skills are required.
    Excellent attention to detail is required.
    Good computer skills, familiar with multiple software program including Windows based programs.
  • Experience in home care is preferred
