The purpose of this position is to perform client care duties based on approved treatment plans. Must be able to observe and report abnormal occurrences or changes in clients to the supervisor and document care provided.


  • Performs visits as prescribed in the physician’s plan of treatment and in compliance with state and federal guidelines. Makes skilled observations regarding patient’s physical and mental condition. Provides patient care and education, monitors patient status, responds to treatment/care, reports changes in patient’s condition, completes and submits appropriate and timely written documentation.
  • Participates in coordination of services with other disciplines and community resources.
  • Participates in case conferences, attends in-services, and/or special case orientation.
  • Demonstrates sensitivity to patient and/or family needs, customs or feelings.
  • Completes and submits all required documentation in a timely manner.

Graduate of an accredited school of nursing

Currently licensed to practice in Indiana and/or Ohio (must obtain both within 30 days of hire)

One year nursing experience required

Must obtain and maintain current  CPR certification 

Proof of eligibility to pass medications required
