Responsibilities

This individual provides physical therapy services under the direction of the Physical Therapist and according to the physician's plan of care.

  • Provides direct physical therapy according to directions of Physical Therapist and in accordance with the physician's plan of care.
  • Treats the patient to relieve pain, develop or restore function and maintain maximum performance.
  • Directs and aids patients in active and passive therapeutic exercises, muscle re-education, gait and functional training, transfer activities and prosthetic training.
  • Utilizes physical agents, including, but not limited to, heat, cold, water, light, electricity, ultrasound and massage, appropriate to the home setting.
  • Observes and reports to the Physical Therapist and the agency personnel, the patient's reaction to treatment and any changes in the patient's condition.
  • Instructs patients in the care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
  • Maintains accurate and complete agency-required documentation and other necessary records, clinical notes and conference notes which will be incorporated into the patient's clinical record within defined time frames.
  • Participates in Home Health team meetings and inservice education programs.
  • Instructs patient's family/caregiver and other agency health care personnel in patient's treatment regime when indicated and appropriate.
  • Performs duties independently with a minimum of supervision but seeks guidance and direction as necessary for performance of duties.
  • Understands and complies with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA and other regulations and licensure requirements as they pertain to the position.
  • Performs other duties as assigned and/or requested.

Qualifications

Minimum Requirements: 

Experience:

  • Minimum of 2 years experience as a PTA in an acute/home care/rehab setting.

  • Must be a licensed driver with an automobile that is insured in accordance with state and/or agency requirements and is in good working order.

Licensure: 

  • Current California license as a Physical Therapist Assistant.
  • Basic Life Support (BLS)
  • Drivers License
  • Car Insurance
