Responsibilities

The incumbent provides timely, accurate, and complete pharmaceutical care which includes reviewing medication orders, performing appropriate clinical interventions with prescribers, and providing drug information. Demonstrates competency in patient care for patient population served. The incumbent will also effectively maintain control of daily pharmacy operations and supervise personnel.

1.1 Investigates and completes Medication Event Report. Full Time: Exceeds (3): greater than 2 per month. Meets (2): 1-2 per month. Does not meet (1): less than 1 per month. Part time and per diem will be pro-rated.
1.2 Follows High Risk Medication policies & procedures including appropriate documentation. Exceeds (3): procedure is followed, physician is contacted if patient does not meet criteria and documentation is completed. Does not meet (1): procedure is not followed and/or documentation is not completed.
1.3 Effectively maintains control of daily pharmacy operations and supervises personnel. Exceeds (3): consistently aware of the needs of the department and takes specific action to direct staff to meet those needs. Meets (2): directs staff based on daily work flow. Does Not Meet (1): does not consistently provide direction to staff to meet the daily work flow.
1.4 Number of orders verified compared against like shifts: Exceeds (3): top quartile. Does Not Meet (1): bottom quartile.
1.5 Number of clinical interventions compared against like shifts. Exceeds (3): top quartile. Does Not Meet (1): bottom quartile.
1.6 Achieves annual performance goals. Exceeds (3): Achieved greater than or equal to 90% of goals. Does Not Meet (1): less than 80% of goals.
1.7 Keeps informed of departmental process changes through documentation of reading minutes, huddles, emails and other communication within one month of information posting or due date assigned. Exceeds (3): greater than or equal to 90% documentation. Does Not Meet (1): less than 80% documentation.
1.8 Completes Quality Assurance logs as required. Exceeds (3): greater than or equal to 90% documentation. Does Not Meet (1): less than 80% documentation.


Qualifications

REQUIRES:

  • Current California pharmacist license.
  • Demonstrated knowledge of pharmaco-therapeutics, therapeutic drug monitoring and therapeutic interventions, pharmaceutical supplies, drugs, chemicals and biologicals, federal and state regulations relating to pharmaceutical transactions, pharmacokinetics, supply management and inventory control.
  • Demonstrated ability to enforce formulary decisions of the medical staff, maintain accurate and complete records, provide instruction and preceptorship on the subject of pharmacotherapeutics, its use and application. Effectively communicate, both written and verbal.
